Dentistry oh Monday morning Dentistry thank you goodness close enough close enough yeah that's good welcome to the show Erin how you doing thank you doing good doing good it's been a while I know it's been a while we saw each other down in Texas at at the dental success Summit and uh caught up real quick but I reached out wanted to bring him back on the show because I it's been a while since we've chatted so I'm excited to have you on the show today yep

good to be here yeah man why don't you tell if if uh to our audience that's listening give them an update on on who you are and what you do in in dentistry and we'll start with there okay who I am and what I do Aon Nicholas um I graduated dental school before most of your listeners were born so 1987 Ronald was President it's been a while the internet didn't exist etc etc um I was in I opened up my scratch start right out of school

and was in practice for 35 years 36 years and sold in 2020 um Grant and yeah yeah it's it was a good move uh anyway I worked back for my old associate I treated him right so we're we're still good uh couple days a week and then somewhere along that journey I had uh run into um uh Mark cus and devil success Institute and started working with them started coaching with them and he is the guy who encouraged me to start helping docs out if I

was interested and I was and so we now have online courses we have Hands-On courses um I speak uh fairly regularly and uh that's kind of my my uh my emphasis right now as well as I've been doing um a mission trip Dental mission trip every year for the last 27 we taking impressions in porn models and doing relines and you know and trimming and polishing Dentures so yeah you'd be very useful I can do all that um I'm probably a little rusty it's been maybe

six or well longer than that it's been like eight or nine years since I was at the dental lab but that's what my entire family does my dad still owns the dental lab my my younger brother runs it I'm sure I could brush up my dental lab skills real fast yeah I I said we we have kids that come along that are like 14 18 and we teach them lab stuff and they a lot of them that's where they work when they're down there for the

week yeah I had to talk to you because that's like part of I talked to my wife so she used to be a dental assistant as well um not as well she used to be a dental assistant and uh one of her dreams is to go and do a mission trip like what you guys do so and I keep telling her about what what you and Mark do and she's like man like can you just set that up and we can like go do that I'm

like yeah I probably should probably should talk to you guys more on yeah we probably get you we can probably get you hooked up yeah I'll do reines and models that's easy yeah can can you actually do the reine in the mouth are you have you I could do a Rel can you yeah oh yeah we might need to bring you along you sound very useful yeah I can be useful down there I'm only useful if you want to take X-rays I used to be an

X-Ray tech but it's a little different it's a little different yeah yeah yeah no that's really cool that you you do that Erin I think that's uh something that I've always wanted to do with my wife you know um with you guys so I I I really need to organize that better I'm not a very organized person sometimes so so so Aaron can I ask on that front because we hear a lot of in uh we obviously we talked to Mark and he brings up the

the mission trips but I love to hear the why you love doing it um what what brought you to decide hey this is something I want to do to the point where you're organizing a trip not just attending what what makes it meaningful for you so for me when I was in dental school one of my instructors um had gone down and he started the trip and he would bring students down and so I'm in my third year and I'm looking at what I've done and

I didn't feel like I had done nearly enough extractions and I knew this mission trip was going down there and I've been I've been involved with other stuff at my church um and so I thought well this would be kind of a cool thing to do if I can get on this and so I did and then you I mean you just take out teeth by the boatload you know so I took out a whole bunch of teeth got a whole lot better at it um

I went the second year also which was my senior year and then I actually took a break while I was building the practice because I didn't feel like I could leave it for uh that long and then eventually I felt like well it was 10 years out from school and someone contacted me about if I wanted to donate to the trip and I was thinking you know I always said I was going to go back and if I don't go back now I'll probably never go

back so I went ahead and went back the practice didn't fall apart what do you know and then uh you know and then I've been going back back pretty much every year since I love that I love that so extractions by the boatload I would love to know what that means that means so we have we have four departments now so we have extractions we have operative Dentistry um we have cross and we actually have a Paro Department believe it or not um and so we

also work with local Dominican dentists so when we first started going down there there weren't LED headlights and loops and so we' get from the local um uh from from the local uh church youth group to hold flashlight flashlights Y and they would hold flashlight force and then a lot of them got interested and some of them went through dental school and so when we come down they come back out as dentists now and they work doing like operative they prefer to do operative Dentistry because

pretty much all they do all day long is is shove teeth you know so um they come back and they tend to be our operative Department um as well as helping with translating and etc etc so it's it's sort of full circle which is really cool and so it's a it's a Cooperative effort between the two groups um and it doesn't exist without either group so that's that's worked out very nicely yeah that's interesting so shifting gears I want to know more about uh I I

butched the name when I first announ introduced you Monday morning dentistry and how you're helping practices with with this sure sure so when I was when I first started my practice you've got to remember again no internet you found something else out it was you it was in a magazine and it was you know months old and I opened my practice and about a year and a half later I never heard of a dental consultant uh now you you you can't throw a dead cat without

hitting three of them but back then I just never heard of it and I was looking at the money in the bank and looking it was going down down down and the practice wasn't building up that much and that's back in the day when you didn't mark it because that wasn't a good thing to do and so yeah I had a Yellow Pages ad and I satad on my butt waited for people to walk in the door you know and so I got to a point

where I was like you know I'm probably going to go bankrupt and I talked to the doct next door to me who was a specialist and we had opened up the same day just pure coincidence and all she had to do was go out and do a lunch and learn with the local um you know General dental office and they would send patients so that was kind of her marketing and she would do that sure yeah and but we both had a lot of free time

so we were talking about it one day I said yeah I'm probably another seven months and I'm going to be gone I just don't see it's picking up real well and she said you know there's guy in the next town over and he hired these consultants and you know maybe that would help you out so I went ahead and I talked to their consultant we started implementing stuff in in place in the practice and in about six months we ended up being break even and stting

to make a little money you know and then things kind of Grew From there um I'm trying to remember why I told you that story what was the question we were talking about how you got started with uh Monday morning dentist so as a as a part of that Consulting Group they offered free CE okay now back that now all the doctors know oh I there's a lot more to learn I need CE back then we had just come out of D school we were like

I'm good you know and I was like ah I don't know if everyone want to do CE and the guy's like it's free you need it for your license you should come and so I would go and I I went to a lot of different courses and some of them were like this is great stuff why didn't they teach this to me in D school and I implemented right away and other stuff was just so fluffy or you would have to change everything you're doing to

implement it that it was basically useless so I always promised myself that if I ever ended up teaching courses that it would be in such a way that you could take it back after the course and put it in place on Monday morning and it would work and that's why the the business is called Monday morning Dentistry that's love it yeah and what do you guys focus on helping helping practices is it is it dealing with the issues that you just you you dealt with at

the beginning of your career or or what's your core Focus yeah for so for us the core focus is more clinical um okay it was really business stuff that helped me get the practice out of the red and into the black but as as I've worked and kind of seeing what happens as you build a practice and now that I've been coaching for um uh for DSI um I can see what problems young Associates have and young doctors have and it's an effort to to help

solve some of those problems so early on I didn't have any way to be I mean we were going bankrupt there was no way I was sending work down the road so I learned how to do mol canals because I sort of had to I had plenty of time um and remember there's no rotaries at this time so they were to totally hand filing and we did we did molo canals and I kind of learned how to do that so later on that was just a

part of my repertoire and as I'm meeting younger docs they like yeah they didn't really teach us that in dental school we just send those out and I'm like no no no no don't send them out hang on let let me help you out I can I can talk you through this you know and so I originally made some online courses which worked out great and then one of those young docs looked at me and said said okay when's the Hands-On course and I was like

oh um six months I'll have it up in six months and so we started doing Hands-On courses and that's been that's been really popular um we also put on a surgical extraction course online and then recently I have found a place in the States because I didn't want everybody does it in Dominican Republic and Guatemala and stuff like that I wanted to do it in the states we found a place in the states where we can we have a patient population that needs the work and

can't afford it and where we can get visiting dentist license uh licenses and so we can now hold this course in the states without having to go out of the country so we're going to kick that off for the first time in Okala Florida in October so I am psyched really psyched about this yeah that's awesome so that so your core focus is is helping practices um with Mo Muller root canals because they they ship that I didn't know they they a lot of uh practices

would Outsource that to another clinic yeah a lot a lot of docs do sometimes the docs have tried doing it they' run into the same problems over and over they're like you know what I'll just send it down the road um whereas I had I kind of had to work through it just because of you know circumstances and so I did um so yeah that's what are some of the challenges they run into that um because I mean I like I said I've been to the

dental lab a lot I I I don't know who's doing what right and who's Outsourcing to who like what were what are some of the challenges that you see even today right so we put up um I put up surveys just to kind of see where people's heads are at and the top two things are a locating all the canals and B being able to file all the way to the end of the route because if either of those don't happen then it's not going to

be a successful root canal no and you know you do that and maybe you separate a file or you have some problems and then you have to have that conversation with the patient you have that conversation a couple three times and you go I don't ever want to have that conversation again no they can go to the endodontist and I'll just put the crown on it when they come back so the way we teach it is actually to both those things at the same time because

it's actually easier to do them at the same time than to do them separately sure and then that way everything stays in house yeah that's really interesting didn't realize a lot of practices were sending those out makes sense if especially if they're not trained on it all the details of that you know with with MERS goodness um all right so I'm curious how did you meet with with Mark cosy and you were you you mentioned earlier before we started recording you were one of his first

black belt practices that work with him and and that's kind of a big deal today uh if if you don't know you know what that is with with what Mark coses does it's like the the premium run practice is kind of how I look at it how did you get involved in all that and and you've coached for years uh with Mark I'd like to know that backstory sure so I was I had taken courses on implants and it was taught by a peronist which was

great but he's used to doing it in a um specialty practice where that's the only thing you're doing you know and so just the prep work for it was more than actually putting in the implant and I just got to a point where I was like this has got to go faster there's got to be we have to be more efficient here it takes longer for me to prep this room than it does to actually put the implant in so I went to a course that

specifically um was addressing that issue and there I met Mark and you know we just kind of talked we sort of hit it off at the end he's like hey I'm having my first summit in a month come for free just pay for your plane in your room and you know feel free to go and so I did and while I was there I was listening to kind of how he had things hooked together in the practices that he ran and I'm like I do all

those things but because the way he has them hooked together he gets a better result than I do and I know I can do this but I'm probably likely to stop about halfway through just for you know I'll get there'll be some other shiny thing I want to do you know and and and I'll miss it so at the end it he's like hey we're going to do a coaching group if A's interested and I was like yes that's the answer I will go and be

in the coaching group so I was in the coaching group and we kind of cooked along for a while and I had a fair amount of years in at that point I was just trying to tweak things and uh at one point he said to me you know you don't really need me that much you you kind of know what you're doing and I said yes I know but I need you to make me do what I know I should do and that way accountability buddy

yeah that's that's literally what it was and then after a while longer he said to me you um if you were interested I think you could help a lot of docs out would you be interested in doing something like that I was like sure that would be great um so he asked me to speak at a second Summit um and then after that we started talking about he like okay so speaking is nice but what else do you think you would want to do to kind

of help docs out I'm like well I'm pretty good clinically so maybe that's the way to go and I knew from talking to other docs that they were sending out their mol root canals and I always felt it wasn't that hard and I talked more than a few docs through the process um and they started doing I'm like maybe this would be a great place to start and so that's kind of where the first course came from and it was an online course in video and

I told you how we got to the Hands-On part of that yeah that's cool yeah I love that um so I would imagine one of my questions here is how can a practice owner increase production less time it has a lot to do with uh mol root canals it can yeah absolutely walk us through that Aaron yeah so first off look at everything you're sending out the door yeah and what if you kept it in the door you know what if you learned how to do

it or your associate learned how to do it or you brought a specialist in or something but you're you're literally you're you're marketing you're paying for people to answer the phone you're paying for people to get them in and then you're taking what could be a sale sending it out you know and you're sending it out the door and then the other piece is that if you have young Associates or you're a young Doc and they're trying to build a practice within the practice you know

a lot of their work's going to be like emergency work you know and the patient will come in and if you can't do that you know if someone has been in for three four five years and they hurt it's not going to be a filling you know it's going to be a root canal or it's going to be an extraction what or the other and if it's a root canal it's not very often it's a front tooth because if they saw a big black thing po

building up on their front tooth they would have been in earlier and got the fing gun sure it's in the back you can't see it it's a little bit you know I'll dig the stuff out of it it's fine um so it's going to be a m r can now or it's going to be a tooth that needs an extraction and usually that means the Crown's all broken down it's below the gum line the gums starting to craw to crawl over top of it and you

know you need to be able to do that extraction if you could do either of those things then once you get the patient out of pain very legitimately you can look at them and go hey glad we got you out of pain but I see some other things that are fires that are coming and you know if we could get you back in and kind of let's get a game plan so that we're not just putting out fires you know they they now trust you they

met you they met your staff they like the way things work and they're more willing to come in and start to continue treatment as opposed to you bring them in you go oh yeah you need a cical extraction I'm I'm going to send you to my oral surgeon he's a really good guy you should do that and then let's come back and let's get a game plan together they never come back yeah okay they just don't that's real complicated and it's yeah when you got them

in your office might as well come up with a game plan from the get-go because it's everyone's busy Y and and from my from an experience standpoint like that's no patients don't want to bounce around right locations you know that that I can imagine that being a pain point in and of itself yeah I a friend that calls it referral fatigue it's like you know like they go here they go there and you pretty soon they're like I don't want to meet anybody else I don't

want to have to you know I just want to get my stuff done um you know the the other piece of that is you know when you're looking at being more more productive you know if you can do more work in a chunk of time then you're going to be more productive obviously right yeah so you know if they need a root canal and they need an extraction and they need two fillings and they need some scaling um you can bring them in for each of

those things separately or maybe you do some and not the other or if you bring them in numb up the lower left and you do all of those things at one time it you're you're saving a ton of time because you're not having to renum that area over and over you're saving a ton of money because you're not resetting the room over and over and paying for somebody to do that and you're just sitting down and doing the work and then you start getting towards quadrant

dentistry and whole mouth Dentistry because you're you're honing those skills and knowing how to do that so if you never learn those skills it's say oh I can do that quadrant but I I can't do the root canal um and I can't do that Cal extraction and I can't you know it's like now we you've bumped yourself out of the game you know and need to be looking at doing things a little differently no this is I like that approach I like that approach to systematizing

um the clinical side of it too because a lot of who we talk to you know being a software company that focuses on the business um we talk about how do you operationalize you know your your staff or your marketing or your your you know Revenue management your accounts receivable all that stuff right but you're saying that the operational processes of how you actually perform clinically can improve your efficiency and there's there's a lot to learn there as well I think that's that's awesome awesome to

hear yeah absolutely so when you look at procedures where you have lots of bits and pieces to set up to have things ready um that can take just a very few minutes or it can take 15 minutes 15 minutes is way too long so say a m Canal we set the room up for a m canal and a crown because we do those procedures together um we have carts that things go on that when there's little bits and pieces so you just when the cart's set

it sits over on the side in the parking lot and when it's in the parking lot we know it's clean it's restocked it's ready to go all you have to do is pull it out slide it into the room it's good okay so for us to set up for a rot Canal building Crown takes seven minutes okay and we know this because we've timed it over and over and over again okay and we have a template and everything goes in a particular spot on the delivery

and in the room so when you when you come into the room you don't have to worry about you know did did Claudia uh set the room up or did Maya set the room up you know exactly where everything is because everybody sets the room up the same way every single time okay and then you then that's going to save you just a ton of time as you're going through the process because you're not looking for things or or oh gee we don't have it oh

it's in the drawer you pulled the drawer out oh somebody didn't restock the drawer okay someone needs to run down the hall thing yeah you know and you can burn a lot of time just looking for stuff yeah well not only that but your your patient's hearing all of this it's hearing like where is this why where did that go and and that that affects the patient experience big time I I had a dentist once that I felt like every time I went in there it

was his first time being in in a month right he was like where is everything yeah he wasn't my dentist very long um and and most of it was like I go in you know before work and I'm taking time off and you know I know I got XYZ that needs to get done it should take 30 40 minutes tops and I'm in there for an hour and a half most of it's while they're looking for stuff I'm not interested in that yeah yeah I don't

think any patient is reminds me Erin when I was at the dental lab we we had similar problems right because there's all these tools that we and processes that we do to create like a denture or a crown or whatever it may be and I remember first getting into my my dad's dental lab and it seems so chaotic like yeah I need I need this this tool to do this proced this uh create this restoration and things were kind of scattered and we we kind of

implemented a very similar process where the bench was set up a certain way for a certain process right we documented we timed everything so that we could be efficient because the the on the dental lab side it's this it's very similar conundrum like oh if my tools and my my everything I need to to create this this Crown is scattered which happened all the time because nobody set up things the right way and we're moving Tools around it's like no no this is the dedicated bench

for waxing crowns right waxing gold CR so you've got all your waxing so we set up these these the benches and the processes very similar and that really helped a ton with our own internal process which I think is so important because we would miss like due dates because things were like disorganized and that's not a good experience for the patient either on the lab side yeah so on that on that Aaron um digging into that a little bit more what do you think like the

end result for uh you know a practice would be by implementing these kinds of processes um I love to know what the end of the rainbow looks like yeah you know building obviously it makes you more efficient which means you're going to be able to see more patients we we talked a little bit about the Loyalty of patients by getting in there and and not just farming them out to other people but what do you see when when you're teaching these courses and dentists start implementing

these processes what what results do they end up getting for their business and for their practice Yeah I think you can I think you can reasonably get easily a 30 to 50% increase in production by making sure you have all this stuff figured out I read an article recently by Roger Levan who's one of the yeah okay so you know who he is I and he said just by knowing how long a procedure takes you can increase your productivity by 30 to 50% because now you're

not wasting time in between and that's just by knowing how long it takes we're not even talking about what you're going to get from systematizing it systematizing your room setups you know all those kinds of things that's just from that one little piece and there's four or five more pieces you can bolt onto that you know that's going to that's going to make it better um I remember in in dental school as was the oral surgery and he said you know when you're going to look

at taking it to that you want to go okay this is how I think I'm going to take it out if if it goes wrong this is the thing I'm going to use to take it out if that goes wrong then this is the so you have all of your wh ifs he says then bring all of those instruments into the room he said don't open them up but have them in the room there's nothing worse than if you were to end up in court and

the patient keeps talking about well then they had to run down the hall and get this and then they had to run down the hall and get that and then they you know so not only from a um efficiency and productivity standpoint but in some ways just from sort of a legal standpoint it's a it's a lot better if you kind of have all that have all that stuff together and and just for a dentist it's better for you to know where you're going if you

have an issue so a lot of times people say oh surgical extraction about a half an hour shouldn't take a half an hour because if you're working on something and it isn't working go to go to plan two if that doesn't work go to plan three and then you know usually you'll end up you know about three and it's about the most you'll need to go because you're very efficient at figuring out this isn't working I'm not going to push something that's not working I'm gonna

go to to plan B you know and then keep working through it that way I love that um Aaron changing gears just a little bit um with all this being implemented one of the things we mentioned we've kind of glass glossed over it is buil using these these patients that come in the emergency patients the root canals the extractions as a way to build patient loyalty and getting them back um on plans and and building those game plans but a lot of the dentists that we've

talked to have said something similar to what you said earlier where they come in the door they don't like emergency patient because they don't they can't retain them um what what would advice would you give to to a dentist or a practice owner uh whether it's how you train your staff or whatever that helps them take these patients that you you you spend all this time and effort getting them in the door they've chosen you you have this golden opportunity how would you advise them to

to build a retention model around that sure um so first off um all the statistics say that 50% of the adult population the United States does not have a regular dental office okay so when these people are coming in on emergency they're not the ones that are coming in every six months okay they're a part of the other 50% so they come in they come in and they have a problem and and now usually they're not coming in all the time because they don't have the

Habit they're afraid I think afraid's pretty big on the on the list um there may be some Financial things going on also okay so somehow in your first contact with them you need to solve all those problems okay to make them feel like oh these guys are different and they're going to take care of me so one of the we we do a lot of mailers and one of the things we put on the mailers every time is got a toothache call today be seen today

real simple it's like I'm hurting they're going to make sure I get seen today okay it doesn't mean I'm taking the tooth out today it doesn't mean we're doing definitive treatment today but what it does mean is if you're in trouble I'm your guy come on in you know and maybe that's a couple of x-rays in and a prescription and an appointment to take care of things down the road but here you know and then once you've gone through that process if you make that as

easy on them as possible do you need sedation do you need nitrous you know what what else can we do to make you comfortable and then can I quickly and efficiently do this thing that you need and then they go oh gee that wasn't that bad um oh you want me to come in and so we can put out don't have to do fires yeah this is pretty bad maybe you can help me avoid this and I like you and I like the way that you're

dealing with me and that's the start of trust will it work with every patient absolutely not you know I'd say if you get 50 to 60% you're probably doing really good but that's 50 to 60% you didn't have and 50 to 60% of a person that needs a lot of care because they haven't been in the last four five six years so it's you know it's always a um it's a big win if you can get them to come in and go through a care yeah

awes I think the narrative you're you're you're saying here along with the treatment is the kind of the we're here for you but not that can't just come from you as the dentist that also has to be communicated with the person that's taking that call with your staff is there as they're setting up the appointment and getting that person in for treatment as uh I I'm assuming that is an all Hands-On deck approach when when you get one of these patients in the door as well

it can't all be on your capacity as the dentist to win them over yeah absolutely it's you know Dentistry is a team sport it just is you know when they're calling in you need to talk about verbiage and that sort of thing and it's like hey I have a toothache okay well how long has it been since you've been no hey I have a toothache I'm so sorry to hear that has been going on a long time I mean some level of care and concern empathy

yeah yeah and then immediately okay what are we going to do you know when I teach the root canal course we talked about diagnosis and I say you know the patient sits in your chair like Doc oh my gosh this thing kept me up all night last night like oh man I am really sorry to hear that has that happened before no BL blah okay and then we're into all the the multiple guest questions about hot or cold is it this or that is it that

you know so yes I want empathy on the front end but I want to transition into treatment just as fast as I can because I don't need to hear the whole story but I need to hear enough of it that I can be empathetic and then we can kind of move forward and and let you know I'm gonna take care of you I like that all right eron our listeners are probably wondering where they can find out more information about you and and all your courses

entire course no but I can I can answer questions and do those sorts of things yeah what I'm on your site right now what are some of the main courses that people um uh typically take yeah so the root canal course is probably the one that's been around the longest and most most popular um the extraction course the online one has been around a while and we're just now rolling out the Hands-On one and I think it's going to be a big deal um we'll kind
