This is the question I get more than any other from optometrists: should I keep the panels or build my own plan? For almost every practice the honest answer is both — but you should understand exactly what each one does before you decide how much weight to put on either.

Who sets the price

With a panel, the payer sets the fee, the allowance, and the covered list. With your own plan, you do. That single difference drives almost everything else.

How the money arrives

Panel revenue arrives after the claim, after the adjudication, after the adjustment. Membership revenue arrives on the same day every month, in full, from the patient's card. There is no claim to file and nothing to write off.

Who owns the relationship

A patient on a panel can be steered elsewhere by their employer's next benefits cycle. A member has a direct agreement with your practice. When their coverage changes, your plan does not.

What each one is good at

  • Panels bring volume and patients who would not otherwise walk in.
  • Your plan converts the patients panels serve badly: out-of-network patients, uninsured patients, and anyone whose materials allowance is already spent.

Nobody I work with drops every panel on day one, and I would not advise it. What I have watched work is quieter: build the plan, enroll the patients the panels do not serve, and let the membership revenue grow until you have the leverage to renegotiate or walk away from the worst contract on your list. Leverage first, decisions second.

The compliance line

Your plan is not insurance and should never be marketed as insurance. It is a direct agreement between the practice and the patient. Check the panel contracts you have already signed for restrictions on discounting covered services, and have counsel review your member agreement — the rules vary by state.
